Retatrutide
Retatrutide (LY3437943)
The first triple hormone receptor agonist targeting GLP-1, GIP, and Glucagon receptors simultaneously — delivering bariatric-level weight loss in research settings.
What Is Retatrutide?
Overview, molecular class & development status
Retatrutide (developmental code LY3437943) is a synthetic single-peptide molecule developed by Eli Lilly and Company. It is the world's first triple receptor agonist — simultaneously activating glucagon-like peptide-1 (GLP-1), glucose-dependent insulinotropic polypeptide (GIP), and glucagon receptors.[1]
Unlike semaglutide (GLP-1 only) or tirzepatide (GLP-1/GIP dual agonist), retatrutide adds glucagon receptor activation, which drives direct thermogenesis and hepatic fat mobilization beyond what satiety-based mechanisms alone can achieve.[2]
Receptor Targets & Their Roles
Development Timeline
- 2021: Phase 1 first-in-human study initiated (NCT04881760)
- June 2023: Phase 2 results published in New England Journal of Medicine
- 2024–2026: Phase 3 TRIUMPH program (5 trials)
- 2025: TRIUMPH-1 topline: up to 30.3% weight loss at 104 weeks
- FDA Approval: Pending (NDA submission expected late 2026)
Clinical Evidence
Phase 2 & Phase 3 TRIUMPH trial data
Phase 2 — Weight Loss by Dose (48 Weeks)[1]
vs. Competing Agents
| Drug | Class | Max Weight Loss | Duration |
|---|---|---|---|
| Semaglutide (Wegovy) | GLP-1 | −15.2% | 68 weeks |
| Tirzepatide (Zepbound) | GLP-1/GIP | −22.5% | 72 weeks |
| Retatrutide | GLP-1/GIP/Glucagon | −30.3% | 104 weeks |

Safety Profile & Tolerability
Phase 2 adverse event data — research reference
Adverse events in Phase 2 were predominantly gastrointestinal, dose-dependent, and mostly mild-to-moderate in severity — occurring primarily during dose escalation.
Most Common Adverse Events (8 mg group, Phase 2)
Key Safety Notes
- GI adverse events primarily occur during dose escalation — not at steady state
- No significant hypoglycemia in non-diabetic subjects
- Heart rate increase: +2–4 bpm at higher doses
- No serious pancreatitis events in Phase 2
- Discontinuation rates: 6–16% depending on dose
Reconstitution & Storage Guide
Step-by-step lab preparation reference
Standard Reconstitution
Step-by-Step Procedure
- Allow the lyophilized vial to reach room temperature (~15 min)
- Clean rubber stoppers with 70% isopropyl alcohol
- Draw 2.0 mL of bacteriostatic water into a clean 3 mL syringe
- Inject water slowly down the side — do not spray onto powder
- Gently swirl (no vortex/shake) until dissolved — clear and colorless
- Discard if cloudiness or particles visible
- Label with reconstitution date, store at 2–8 °C
Storage Conditions
| State | Temperature | Shelf Life | Notes |
|---|---|---|---|
| Lyophilized (powder) | −20 °C (freezer) | 12–24 months | Keep desiccated & dark |
| Lyophilized (powder) | 2–8 °C (fridge) | 3–6 months | Avoid freeze-thaw cycles |
| Reconstituted | 2–8 °C (fridge) | 28 days | Do not freeze |
